                                         A Medical Scribe is an health professional who documents the real time  encounters of a doctor and patient. They also provide a complete history of the patient for doctor's which is needed for patient's care. Depending on which area of practice the scribe works in, the position may also be called clinical scribe , ER scribe or ED scribe (emergency department) . A medical scribe's primary duties is to follow a doctor's work day and chart patient encounters in real time using Electronic Health Record (EHR). Medical Scribes free the physician to increase patient contact time , manage patient flow through the department and increase productivity to see more patients.

                                   


 The introduction of EHR or Electronic Health Records has revolutionized the practice of medicine.  An EHR is a digital version of a patient's chart . EHRs are real-time, patient centered records that make information available instantly and securely to authorised users. While an EHR does contain the medical and treatment histories of patient's, an EHR is built to go beyond standard clinical data collected in a provider's office and can be inclusive of a broader view of a patient's care.  An can:

Duties of scribe includes:
  • Scribe works with one physician per shift.
  • Scribes documents the physician dictated patient history , including history of a patient's illness, reviews of system, past medical and surgical history, family and social histories , medications and allergies. 
  • When the physician concludes the patient's encounter, the physician will review all documentation completed by scribe, make any necessary amendments and sign the chart. The physician is  ultimately responsible documentation of patient's encounter.
Skills Needed to be a Medical Scribe includes:

                                An ER Scribe works in the emergency department of a hospital. Their duties may include overseeing the documentation of each patient's visit to the ED and acting as the physician's personal assistant.
